Introducing our new Care Co-ordinator and Carers Champion

We are delighted to introduce a new role within the practice team – Care Co-ordinator, working within our Health and Wellbeing Team.

Susie was previously a Health Coach at the practice, and has returned in a new role focused on supporting vulnerable patients.  She is also our Carers Champion.  She explains her role as follows:

I act as a central point of contact for vulnerable patients, including frail and elderly individuals and those living with long-term conditions, to ensure you feel supported and have access to the services that are right for you. I work closely with the GPs and practice teams to support you in understanding your condition and to ensure your changing needs are addressed.

I am also fortunate to be the Carers Champion, acting as the surgery’s main point of contact for carers. You may be unaware you are a carer as you may see yourself as a husband, wife, daughter, son or friend.  My role is to identify carers, ensuring you are aware of the entitlements, benefits, information and support available to help you make your life easier and that of the person you care for. Whilst caring for someone can be very rewarding, it can also impact on your own physical and mental health, and here at the surgery we offer annual health checks for carers.
