If you are going on holiday and think you may need travel vaccinations then you must contact the surgery as early as possible so that we are able to provide you with everything you need in plenty of time.
Please contact or visit the surgery to obtain the questionnaire for completion.
Please return the completed documents to the Health Centre for the nurses to research the vaccinations needed for your destination. They will then contact you to arrange an appointment if needed.
Although most travel vaccinations are given free of charge, there are a small number that have to be given privately and a charge will be made for those vaccinations. Bruton Surgery is a Yellow Fever vaccination centre.
You will be told if you have to pay for any of the vaccinations you need and, if so, you will be required to pay for the course in full prior to the first vaccination. In some instances we may need to direct you to other services to provide the non-NHS vaccines, the nurses will explain all of this to you once they have reviewed your information and will then contact you via telephone.

It is important to complete this form as early as possible – at least 6 weeks before you travel – as an appointment may be required to administer travel vaccinations.
Private vaccines can be administered at the surgery but please note there will be a charge for any private vaccines. Bruton Surgery is a Yellow Fever vaccination centre.
Please remember that the vaccines will need to be administered at least 2 weeks before you travel to allow for them to work.
Yellow Fever
Bruton Surgery is a Yellow Fever vaccination centre, and can provide Yellow Fever vaccinations if required for travel purposes for Bruton patients.
Please note that this is not an NHS vaccination, therefore charges will apply for the vaccination including certificate. If you require a replacement certificate the charge will be £20 per certificate.
