Healthier You: NHS Diabetes Prevention Programme

Many cases of Type 2 Diabetes can be prevented by making lots of small changes to your diet and lifestyle.  If you are at risk of developing Type 2 Diabetes, the good news is that by learning more about it and implementing small changes to your diet and lifestyle, you can prevent it occurring in the first place.

The NHS Diabetes Prevention Programme provides support either through telephone sessions, and interactive app or an In-Person Group programme.  You will be provided with the tools and guidance to make small but significant changes to your lifestyle to not only reduce or possibly reverse the risk of developing Type 2 Diabetes, but also other conditions such as heart disease and stroke.

You can self-refer, but will require some medical details such as your NHS number and recent blood test results.  This information is available on your NHS App, or please contact our Healthcoaches who can provide the information to you.  To self-refer, please visit: Registration form – Preventing Diabetes (preventing-diabetes.co.uk)

A Patient Information Leaflet is available here: NDPP Patient Information Sheet May24.

A Patient Information Booklet is available here: NDPP Patient Info booklet.
